Abstract [eng] This dissertation describes the long-term results of the beating-heart transventricular mitral valve (MV) repair. A comprehensive scientific evaluation of the new treatment method is presented and is relevant to cardiac surgeons and cardiologists worldwide. Although the operation had been proven safe and effective in treating degenerative mitral regurgitation (MR) in previous studies, the postoperative follow-up data of 100 selected patients has shown that recurrent MR was increasing over time. A comparative analysis of various demographic, echocardiographic, and perioperative indicators was undertaken. Causes of recurrent MR and prognostic factors have been identified for refinement of the patient selection criteria. An intraoperative over-tensioning of the repaired leaflet segment was found to prevent elongation of the artificial chords and recurrence of greater than 2+ MR at the long-term. Having performed the mixed-effect continuation model analysis, the probability of recurrent MR was estimated. The most recent literature on degenerative MV disease and its treatments has been reviewed in order to provide the reader with a scope of repair durability in contemporary mitral valve surgery.
