Abstract [eng] In many European countries as well as Lithuania the prostate cancer (PC) is one of the most frequently diagnosed male oncological diseases; it is also one of the main causes of death among men in Lithuania. Despite improving diagnostic and therapeutic alternatives, the survival of PC patients is still low due to late diagnosis and relapse that goes in some cases undetected. The PC treatment is sometimes provided in cases when only observation is necessary. Therefore, the differentiation of clinically significant PC from the non-significant (cases that do not have lethal outcomes) is of crucial clinical importance. This scientific work was aimed to evaluate the significance of non-invasive research methods (3D ultrasound imaging system or histoscanning, multiparameter magnetic resonance imaging, transperineal biopsy, and molecular markers) in identifying clinically significant PC cases. The comparison of these methods revealed that the most accurate procedure was multiparameter magnetic resonance imaging. Among the molecular markers (CRISP3, LMTK2, MSMB), the highest accuracy was characteristic of CRISP3, while LMTK2 enabled the best differentiation of clinically significant and non-significant PC cases. The study findings enable more precise diagnosing of clinically significant PC in the clinical practice.
