Background. Riedel‘s thyroiditis is a chronic inflammation of the thyroid that tends to progress to the destruction and fibrosis of the thyroid gland. It is a rare disease with an estimated incidence of only 1–1.6 cases per 100 000 people. This disorder usually affects middle-aged women. Aim. Based on the latest scientific literature, to analyze and review the knowledge on the etiology, diagnosis, treatment and prognosis of Riedel‘s thyroiditis. Materials and methods. The search of the literature was performed in PubMed and Wiley online library databases using the keyword “Riedel’s thyroiditis” and its combination with the keywords “etiology”, “diagnosis” and “treatment”. 22 publications published in 2018-2023 were chosen and reviewed. Results. The etiology is not definitively known. It is suggested that it might be a form of autoimmune thyroiditis, a local manifestation of multifocal fibrosclerosis or an immunoglobulin G4-related disease. Symptoms are nonspecific. Laboratory tests usually reveal hypothyroidism and positive anti-thyroid antibodies. Imaging tests are important for the evaluation of metabolic activity, stiffness of the thyroid gland, and extension of fibrosis to extra-thyroid regions. Biopsy and histological investigation are mandatory for the confirmation of diagnosis. First-line treatment are medications (glucocorticoids or tamoxifen); when obstructive symptoms occur, patients undergo surgery. If treated, disease remains stable for years. Conclusions. Riedel‘s thyroiditis is a rare disease of the thyroid gland. Due to its scarcity and lack of experience, clinicians often encounter diagnostic and management challenges. It is widely accepted that histological examination is mandatory for definitive diagnosis confirmation, and the mainstray of medical treatment is glucocorticoids. |
