SUMMARY Requirements of hospital care and nursing features in patients with intestinal stomas Master Final Work by Nursing Department Student Turba Ž. Research Adviser–Docent Simutis G. Vilnius University, Medical Faculty, General Medicine Practice and Nursing Department Vilnius, June 14, 2006 The problems of patients with intestinal stomas and the requirements of their care after surgery are the major questions in the nursing nowadays. Aim of the investigation: to define the hospital care needs and nursing features for patients who underwent surgery with the formation of intestinal stomas. Tasks of the investigation: 1. To define problems during early postoperative period after stoma formation using the method of survey. 2. To analyze the expectations of patients after stoma formation. 3. To evaluate if the nursing services are efficient for patients after intestinal stoma formation. 4. To define nursing features after intestinal stoma formation. 5. To evaluate the competence of nurses in providing care for patients with intestinal stoma formation. The methods of survey using two questionnaires and statistic analysis of documents were used. 30 patients with intestinal stomas and 64 nurses of surgical departments in Vilnius hospitals were questioned. The analysis of 73 cases was performed. Results and conclusions: 1. The most common problems after intestinal stoma formation are fear and concern with the quality of life (70%), postoperative pain (67%), lack of knowledge of stoma care (50%). The problems of surgical wound and stoma care are more frequent for patients with permanent intestinal stomas than temporary intestinal stomas. 2. Lack of attention to patients psychological rehabilitation (8%). Some patients would prefer more respect (40%), attention and time for socializing (20%) as well as moral support (7%). These problems are more common to people who have university degree or higher education. 3. The lack of postoperative patient teaching (27%). Most patients need more information about stoma care (93%), they would like better teaching (43%). Two thirds of patients do not get enough information on feeding after operation (73%). One third of patients do not know what kind of stoma they have (30%). Most patients lack information about social life having a stoma (57%). 4. Two thirds of patients need further nursing care at home (70%), especially patients with permanent stomas. According to nurses the main nursing features are: skin care around stoma, proper use of stoma care products (89%), the teaching of personal care after operation (66%), Information on daily life (30%). 5. The nursing problems after intestinal stoma formation are: lack of time due to high work intensity, lack of psychological knowledge and stoma nursing features and lack of stoma care products. |
