Abstract [eng] Purpose of review: To analyse the main methods, factors contributing in choosing the right method, results and complications of breast reconstruction after mastectomy. Methods: A review of scientific literature was conducted using PubMed, Google Scholar, and Researchgate databases. The following keywords and their combinations were used in the advanced search: breast reconstruction, mastectomy, implants, autologous tissue, radiotherapy, complications, results. Results: Breast reconstruction may be performed using either autologous flaps or breast implants. Currently, implant-based reconstruction is the most common method. Implant-based reconstruction can be done epipectorally, subpectorally or using dual-plane technique. Breast reconstruction with autologous flaps is favoured method because of better patient satisfaction, more natural looking breast and sensation. Autologous reconstruction is suitable for patients with larger or average-sized breasts and a sufficient amount of tissue in donor sites available. In the literature, the deep inferior epigastric perforator flap (DIEP) is considered the gold standard of autologous breast reconstruction. Breast reconstruction can be immediate and delayed. Delayed reconstruction has better aesthetic results, but also a higher risk of postoperative complications. Conclusions: There is no universal breast reconstruction method that is suitable for all patients. In each case, the reconstruction method is chosen individually, taking into account the current criteria. It is important to understand each patient's expectations, evaluate their feasibility, and choose a method that can provide the best possible outcome. Radiotherapy is significant part of cancer treatment that negatively affects the results of reconstruction, majorly increasing the rate of complications.
