Abstract [eng] Cystic pancreatic neoplasms are an increasingly common pathology in clinical practice. The origin, course, and likelihood of malignancy of cystic neoplasms are highly variable, and differential diagnosis and risk assessment of cystic neoplasms are important. In this manuscript, a clinical case report and a literature review on the diagnosis and management of cystic neoplasms are presented. A 61-year-old man presented to our institution with diarrhoea, intermittent light-coloured stools, and abdominal pain. Radiological examinations revealed a solid, calcified mass of 53x30x35 mm on the mesentery of the small intestine and a cystic pancreatic mass of 56x40 mm with no dilatation of the main duct of the pancreas, solid masses, or calcification. Cystic neoplasm of the pancreas was suspected. An endoscopic ultrasound with aspiration of the cystic fluid and cyst wall biopsy were performed. The results of cyst fluid analysis indicated a serous cystic neoplasm, but the results of a biopsy of the cyst wall indicated that the mass was not serous. The previously mentioned solid mass in the mesentery was resected and histopathological analysis showed a well-differentiated fibromyxoid sarcoma, while the final diagnosis of a cystic mass remains unclear. Clinical, radiological, biochemical, and pathohistological features of the diagnosis must be considered while differentiating between the various forms of cystic pancreatic neoplasms. A purposeful evaluation of the patient would help to prevent the progression of malignant cystic neoplasms and ensure optimal patient care without excessive treatment or burden on the patient.
