Abstract [eng] Author. Miglė Filipauskaitė Academic supervisor. Prof. dr. Valentinas Uvarovas The aim of research work. To review the indications, outcomes and complications of primary total hip arthroplasty after acetabular fractures, using research studies and case report. Methods. The literature search for this work was performed using the PubMed database. Articles were selected using keywords and their combinations: acetabular fracture(s), total hip arthroplasty, emergency primary hip arthroplasty. Selection criteria were used for the articles: full, freely available text in English, publications of the last 10 years, recent articles were more valued. Exclusion criteria: articles about femoral head and neck fractures. Applying keywords, 348 articles were found, after reading the titles and abstracts, 40 articles were selected, after reading the full text 22 articles were used. Case report. The patient suffered an injury falling from a ladder in 2008 - fracture of both pubic bones, injury to the right acetabulum and pelvic ring. Urgent primary one-stage total hip arthroplasty with osteosynthesis of the posterior pelvic column with intraacetabular plates was performed. The patient's condition was assessed in 2023 using the Harris club score (good result - 83 points), the quality of life questionnaire (no activity limitations, social functioning is not impaired) and computed tomography (endoprosthesis without signs of loosening). Discussion. It is predicted that the frequency of acetabular fractures will increase in the future - 30 percent by 2030. Hip arthroplasty is more often used to treat hip fractures in older patients, due to the more frequently observed osteoporosis. The most common complications were endoprosthesis loosening and infection. Outcomes evaluated in patients up to 16 years postoperatively were good or excellent using the Harris hip score. Conclusions. The primary indication for emergency total hip arthroplasty is elderly patients with osteoporosis. Primary hip arthroplasty is associated with better results in the treatment of acetabular fractures, as well as lower complication rates due to a shorter hospitalization period.
