Abstract [eng] Introduction and aim: Extrahepatic oncological involvement and various tumor metastases in the liver are known as absolute contraindications for liver transplantation. The first attempts to do liver transplantation with extended criteria were unsuccessful. However, advancement in systemic therapy for cancer treatment, progress in surgical and anesthesiologic techniques and reduction of complications after liver transplantation allow to reassess the possible use of extended criteria liver transplantation to treat various hepatic cancers. The aim of this study is to review current evidence about non-standard application of liver transplantation in order to treat various oncological liver diseases. Materials and methods: A comprehensive literature review was conducted using search terms and restriction criteria in the PubMed database. All titles and abstracts were reviewed. Full-text articles were retrieved if relevant abstracts were identified. An additional manual search of the reference lists was performed in order to ensure the comprehensive literature search procedure. Results: In this comprehensive literature review, 6 studies where advanced liver transplantation criteria were used to treat hepatocellular carcinoma were examined. A review of 4 studies applying extended liver transplantation criteria in the treatment of colorectal cancer was done. The analysis of 3 multicenter studies with advanced liver transplantation criteria in order to treat neuroendocrine tumor metastases in the liver was performed. Moreover, today’s evidence about the use of liver transplantation in the treatment of melanoma liver metastases was discussed. Conclusions: Nowadays, studies show that the use of extended liver transplantation criteria in treatment of oncological liver diseases result in satisfactory outcomes. However, strict selection of recipients and donors, prediction of survival and disease recurrence are essential for the application of extended liver transplantation criteria. Further studies should be done to point out the most appropriate selection criteria in order to achieve the best liver oncology treatment outcomes.
