Abstract [eng] Traumatic brain injury is closely associated with deterioration in cerebral autoregulation function. Maintaining cerebral perfusion pressure closer to the patient-specific “optimal” cerebral perfusion pressure (a derivative of arterial blood pressure and intracranial pressure monitoring data) was postulated to improve neurological outcome after brain trauma. This systematic review evaluates the link between optimal cerebral perfusion pressure deviations and outcomes in the traumatic brain injury population. In April 2021 we searched for eligible studies in MEDLINE, Cochrane Library Central Register of Controlled trials, ClinicalTrials.gov, World Health Organization International Clinical Trials Registry Platform, and Web of Science databases. Studies addressing optimal cerebral perfusion pressure deviations effect on outcomes (i.e. neurological recovery and mortality rates) after traumatic brain injury were included. A net total of 18 studies met eligibility criteria for qualitative analysis. Although the collected data supports an idea that wider deviations from optimal cerebral perfusion pressure might lead to worse outcomes, the lack of quality of evidence and high level of risk of bias in the underlying studies were the main limiting factors preventing the establishment of decisive conclusions. The topic should be studied further in a controlled and well-designed fashion in order to lay an evidence-based foundation regarding the optimal cerebral perfusion pressure and clinical outcomes association.
