Abstract [eng] Aims and objectives. To review the scientific literature and analyse the clinical case; to summarize the diagnostic and treatment alternatives for a well-differentiated liposarcoma of the extremities; to provide a detailed analysis of the challenges in the management of tumor located near to the brachial plexus. To make a recommendation list for the diagnosis and treatment of a well-differentiated liposarcoma in the area of the brachial plexus. Review of the literature. A well-differentiated liposarcoma is a locally aggressive neoplasia of the soft tissue, consisting partially or entirely of the proliferated adipose tissue. Compared to other types of liposarcomas, the prognosis for well-differentiated liposarcoma in the extremities is favourable, and the risk of dedifferentiation is about 5 percent. Magnetic resonance imaging is usually used in the diagnostics, and the main method of treatment is surgical excision. A well-differentiated liposarcoma located in the zone of the brachial plexus is a serious surgical challenge, and there is little literature on this issue. Case report. The surgical treatment of a patient with a long-term history of the tumor in the area of the brachial plexus is described. During the first described surgery, the tumor was excised in a non-radical technique. The pathohistological examination confirmed tumor to be a well-differentiated liposarcoma. Over the course of a year, the tumor recurred. During re-excision a complication occurred – the radial nerve was damaged. Conclusions and recommendations. Well-differentiated liposarcoma of the extremities is a fatty tumor of unknown aetiology, with a low risk for dedifferentiation and metastasis, but a high risk for local recurrence. Magnetic resonance imaging is important in its differential diagnostics. The central method of treatment is surgical, but in the area of the brachial plexus it is technically challenging. The choice of the optimal treatment strategy is complicated by the lack of scientific data. After examining the literature, the list of recommendations for the treatment of a well-differentiated liposarcoma in the area of a brachial plexus was made.
