Abstract [eng] Relevance and aim of the study. The optimal umbilical hernia repair method with best short and long-term outcomes remains debatable. Studies investigating surgical treatment of umbilical hernia solely are scarce and controversial. The aim of this study: to evaluate outcomes after laparoscopic hernioplasty versus open approach in the treatment of umbilical hernia and to present umbilical hernia clinical case. Material and methods. A literature search was conducted in Medline (PubMed) database from 2022 March 12 up to 2022 April 19. Inclusion criteria were publications comparing short and long-term outcomes after laparoscopic and open umbilical hernia repair. The follow-up period for all patients in this systematic literature review ranged from 30 days postoperatively to 3,5 years. Results. A total of 121 articles were identified, 4 of which met the inclusion criteria. All included studies were retrospective, with a total of 33 802 patients, respectively 7 229 in laparoscopic group and 26 573 in open repair. Results from all included studies confirmed that longer length of stay, surgical site infections and non-infectious occurrences, hernia recurrence rate were significantly more common in open repair group. Conclusions. This systematic literature review suggests that laparoscopic hernia repair is associated with decreased risk of wound morbidity and recurrence rate, shorter postoperative length of stay. Case report of umbilical hernia presented in this study confirms that surgical treatment of umbilical hernia in Lithuania meets all contemporary principles and recommendations, although postoperative short-term outcomes are similar as described in scientific literature.
