D Vilnius University, Faculty of Medicine Institute of Rehabilitation, Sports Medicine and Nursing There are many cases in urgent medicine where patients need to be artificially ventilated for prolonged time. This include polytraumatic patients as well as patients with different other pathology. Most of these patients have tracheostomy which had been performed earlier for ventilation purposes. Part of these patients have permanent tracheostomy because of existence various tracheal and laryngotracheal postintubation pathology (stenosis, granulations, malacia and etc.). Life with tracheostomy has many disadvantages including permanent orifice in the anterior wall of the neck, non-physiologic mechanism of breathing, patients have no or very limited possibility to speak. In order to change such uncomfortable clinical status we need to change tracheostomy tube with endotracheal silicone T tube (Montgomery safe T tube). The main advantages of this tube are as follows: patient will be able to breath through the nose; colar opening in the tube is usually pluged up; the patients are able to speak!! Despite the original tracheal T tube is very simple in construction, it needs some time for patients to learn some tricks starting living with a T tube in place. The professional help of nursing staff is very necessary for the patient during that period. The aim of the present research was as follows: to find out nurse`s influence in the management of patients with tracheal T tube; and what is the influence of the nursing staff preparing patient for being with T tube at home. The specific questionnaire was issued to accomplish the task of the study. The questionnaire was provided for the patients with silicone T tube who had been treated at the Department of General Thoracic Surgery. The main points of afforementioned questionnaire were as follows: 1. how the patients evaluate the professional skills of the nursing staff; 2. what are the main nursing-service points dealing the T tube being in a hospital. Totally 76 questionnaire forms have been evaluated. The research data revealed that suction of the sputum from the tube, daily changing dressing material around the tube and management of the skin in the neck around the tube – are the main points influencing general condition of the patient after introducing the safe T tube. The statistic analysis of the data showed statistically reliable connection between patient age and their skils how to manage the T tube. The hypothesis fraimed in this study was proved as a correct. |
