Abstract [eng] Background. Hypothyroidism during pregnancy is a significant health concern that can adversely affect both maternal and fetal well-being. The thyroid gland plays a crucial role in regulating metabolism, growth, and development, and its hormones are vital for the proper development of the fetus, particularly in the first trimester. Inadequate thyroid hormone levels can lead to a range of complications of mother and the newborn. Aim. To analyze and review scientific literature describing diagnostic possibilities, treatment strategies and pregnancy and fetal outcomes on hypothyroidism. Material and methods. A literature review was conducted by searching scientific publications in the PubMed database. The review included articles written in English and published within the last 10 years (2014–2024). Results. Hypothyroidism during pregnancy is associated with various maternal, pregnancy, and neonatal complications. Overt hypothyroidism significantly increases the risk of miscarriage, preeclampsia, preterm birth, and low birth weight. Subclinical hypothyroidism can also have negative consequences, especially if left undiagnosed and untreated. Additionally, maternal hypothyroidism has been found to impact fetal neurocognitive development, leading to lower IQ in children. Although levothyroxine treatment is the gold standard for managing hypothyroidism during pregnancy, individualized care is essential, considering the woman's baseline thyroid-stimulating hormone levels, potential autoimmune disease, and regular thyroid function monitoring throughout pregnancy. Conclusions. Hypothyroidism during pregnancy is a significant health concern that can affect both maternal and fetal health. Timely diagnosis, properly adjusted levothyroxine treatment, and continuous monitoring of thyroid function can help prevent adverse pregnancy and neonatal outcomes.
