Keratoacanthoma (KA) is a rare rapidly growing low-grade squamoid neoplasm that typically arises in sun exposed areas, particularly among elderly patients. Although surgical excision is the first-line treatment for KA, it may be avoided in high-risk patients where surgery might lead to complications or poor cosmetic outcomes. We report the case of an 82-year-old woman with a 30 mm hyperkeratotic nodule in the left temporal area. Dermoscopical and histopathological findings confirmed the diagnosis of a giant KA. Due to the patient’s advanced age, use of anticoagulants, large size and facial location of the nodule, the decision was made to avoid surgery and instead begin treatment with intralesional methotrexate (MTX). Marked tumor regression was evident one week after treatment initiation, following a single MTX injection. After four intralesional MTX injections, the KA nodule had fully resorbed, and treatment was completed after a total of five injections, without any subsequent recurrence. This case report demonstrates that intralesional MTX may offer an effective, safe, and cosmetically favorable alternative for treating giant KAs in patients who are not suitable candidates for surgery. |
