Introduction. The mental health of transgender individuals has received increasing scientific attention in recent years, as this population shows higher rates of depression, anxiety, and other psychiatric disorders compared to the general population. Psychological distress is often associated with social stressors such as stigma, discrimination, and lack of social support. In some cases, obsessive–compulsive disorder (OCD) symptoms may also be present, complicating diagnostic assessment and treatment planning. Clinical case presentation. We present the case of a 17-year-old adolescent diagnosed with gender dysphoria and comorbid obsessive–compulsive disorder. The patient identifies as male, experiences marked gender dysphoria and reports significant family-related social stress. Additionally, symptoms of OCD, moderate depressive symptoms, and elevated anxiety levels were identified. The patient is currently treated with sertraline and is engaged in psychotherapy. Conclusions. This case highlights the complex interaction between gender dysphoria and OCD, as well as associated diagnostic challenges. A multidisciplinary approach is essential in the mental health assessment of transgender individuals, considering both social stressors and individual psychopathological factors. Careful differentiation between OCD symptomatology and gender identity-related experiences is crucial to ensure accurate diagnosis and appropriate treatment. |
