Crystal-induced arthropathies stands for a group of inflammatory joint disorders, which involve microcrystal deposition that causes recurrent episodes and sometimes chronic inflammation. Conventional anti-inflammatory therapies are effective in many cases, however numerous patients experience insufficient response or contraindications, especially in the setting of advanced age and comorbidities. Progress in the understanding of the innate immune system´s part in these diseases have drawn attention to cytokine-driven pathways as potential therapeutic targets. The aim of this literature review is to review available evidence on the use of biological disease-modifying drugs in the treatment of crystal-induced arthropathies. This literature review assesses the role of interleukin-1-dependent inflammatory pathways in crystal arthropathies and synthesizes available clinical data on biological therapies used for this context. The efficacy and safety profiles of interleukin-1 inhibitors, namely anakinra, rilonacept and canakinumab, are assessed based on published clinical studies. Additionally, current limitations in the clinical evidence are identified and subjects that need further research are stressed. The reviewed literature implies that biologic therapies may offer effective treatment alternatives for certain patient groups with refractory disease or contraindications to standard treatment. Nevertheless, limited high quality clinical data restrict definitive conclusions at this point of time. More deliberate clinical trials are essential to further define the role of biological disease-modifying drugs in the treatment of crystal-induced arthropathies. |
