Idiopathic granulomatous mastitis (IGM) is a rare benign inflammatory disorder of the breast, usually characterized by non-necrotizing granulomatous inflammation. It most frequently manifests in premenopausal women with recent history of pregnancy and lactation. The etiology of IGM is currently unclear, though several different theories have been put forth, including an autoimmune disorder, an infectious agent and a hormonal disturbance. The clinical presentation usually includes a tender, palpable breast mass with erythema and swelling. The clinical and radiological signs of IGM are not pathognomonic and have a lot of overlap with those of infectious mastitis and inflammatory breast cancer, which makes the differential diagnosis challenging. There is no consensus on the best treatment for IGM but the most commonly used options are corticosteroid or surgical management. However, recurrences are common after most types of treatment. This paper presents two contrasting cases of IGM (one – a severe, complicated case in a multiparous woman, treated surgically, and another - a case of IGM in a nulliparous woman associated with medication-induced hyperprolactinemia) and a review of current literature about IGM. |
