Total dislocation of the talus is an exceptionally rare injury, accounting for approximately 0.06% of all joint dislocations. Due to the unique anatomy of the talus, including its extensive articular cartilage coverage, absence of muscular attachments, and reliance on a delicate extraosseous blood supply, this injury carries a high risk of serious complications, most notably avascular necrosis (AVN), deep infection, and post-traumatic osteoarthritis. The available evidence consists predominantly of case reports and small case series, reflecting both the rarity of the condition and the absence of larger prospective studies. This thesis provides a structured overview of total talar dislocation, covering its epidemiology, anatomy, classification, injury mechanisms, pathology, diagnostic approach, treatment strategies, and prognosis, based on a focused narrative review of the current literature. In addition, a clinical case of a 22-year-old male who sustained an open anterolateral total talar dislocation during a basketball game is presented. The case is discussed in the context of the existing literature to illustrate how this rare injury presents and is managed in clinical practice. The literature review and case analysis together support the principle that early anatomical reduction and talus preservation are the most important factors in achieving a favorable outcome. Long-term follow-up remains essential given the well-documented risk of delayed complications. |
