Abstract [eng] Within the field of mental health, growing attention has been directed toward therapeutic approaches that combine conventional treatment with creative methods. One of these approaches is music therapy, where improvisation and composition can help patients express emotions, explore personal experiences, and engage more actively in the therapeutic process. Nevertheless, the use of composition in adult music therapy remains insufficiently explored. This study sought to examine the functionality of composition in adult music therapy by analysing the dynamics of composition and improvisational composition processes during individual sessions held in a day hospital setting of a mental health centre. Attention was also given to the theoretical understanding of these methods, their practical application, and their significance within the therapeutic process. The research was based on a qualitative strategy and a case study design. Data were gathered through observation of individual music therapy sessions, analysis of therapeutic reflections, and the use of an adapted composition process assessment tool. Participants were patients attending a day hospital of a mental health centre who took part in individual music therapy sessions. The analysis showed that the composition process changed gradually over time. In the early stages of therapy, patients tended to participate more cautiously, whereas later sessions reflected greater activity and independence. Composition-based work created opportunities to express emotions, develop clearer musical structures, strengthen autonomy, and support the therapeutic relationship. Overall, the findings indicate that composition can be regarded as a meaningful method in music therapy for promoting adult mental health.
