Abstract [eng] Introduction. Chronic obstructive pulmonary disease (COPD) remains a leading global cause of morbidity and mortality, characterized by progressive airflow limitation and significant clinical heterogeneity. Despite advances in pharmacotherapy, COPD continues to be associated with frequent exacerbations, hospitalizations, and high mortality. Recent research has focused on molecular mechanisms and personalized treatment strategies. Objective. To evaluate the pathogenesis of COPD and analyze the effectiveness of emerging therapeutic strategies, including biologic therapies, anti-inflammatory, and antioxidant approaches. Methods. A literature review was conducted using PubMed, Scopus, and Cochrane Library databases. Randomized controlled trials (RCTs), meta-analyses, and guidelines published between 2015 and 2025 were included. 37 sources were analyzed. Results. COPD pathogenesis is driven by chronic inflammation, oxidative stress, and structural lung changes. Triple inhaled therapy reduces exacerbation rates by approximately 25% but does not significantly alter disease progression. Biologic therapies targeting IL-4/IL-13 pathways reduce exacerbations by up to 30% and improve lung function in selected patient populations. Antioxidant and novel anti-inflammatory therapies show modest but clinically relevant benefits. Most innovative treatments demonstrate efficacy primarily in biomarker-selected subgroups. Conclusions. COPD management is shifting toward a precision medicine approach based on phenotypes, endotypes, and biomarkers. While biologic therapy represents a significant advancement, further research is required to improve long-term outcomes.
