| Abstract [eng] |
Introduction. Dermatomyositis is a rare autoimmune disease characterized by proximal muscle weakness and characteristic skin manifestations. Due to clinical heterogenity and overlapping syndromes its diagnosis may be challenging. Dermatomyositis is also associated with an increased risk of malignancy, particularly within the first years after diagnosis, and may present as a paraneoplastic syndrome. The aim of this study was to present a clinical case of dermatomyositis associated with serous carcinoma of the fallopian tube and to highlight diagnostic challenges and the importance of malignancy screening. An analysis of a single clinical case was performed. Clinical presentation, laboratory findings, imaging studies, histopathological data and treatment response were evaluated. A literature search was conducted in the PubMed database using keywords and their combinations: „dermatomyositis“, „paraneoplastic“, „malignancy“, „cancer“, „carcinoma“. Clinical case presentation. A 70-year-old woman presented with progressive muscle weakness, dysphagia, and persistent skin manifestations. Initial findings suggested systemic lupus erythematosus; however, the disease course was atypical and resistant to standard treatment. Myositis-specific antibodies (anti-TIF- γ, anti-Ku) were detected, raising suspicion of dermatomyositis. Despite initialy negative oncological screening, repeated imaging revealed a mass in the greater omentum. Histopathological examination confirmed high-grade serous carcinoma of the fallopian tube. Conclusion. This case highlights the diagnostic complexity of dermatomyositis and its association with malignancy. Repeated and comprehensive malignancy screening is essential in patients with atypical or treatment resistant disease. |
