Abstract [eng] Introduction. Pulmonary arterial hypertension (PAH) is a rare, progressive, and potentially fatal disease characterized by increased pulmonary vascular resistance and right ventricular failure. Despite advances in therapy, the disease remains associated with significant mortality. In recent years, the focus has been on early diagnosis, risk stratification, and individualized treatment. Aim. To evaluate the importance of PAH diagnosis, risk stratification, and contemporary as well as emerging treatment strategies for clinical outcomes. Methods. A systematic literature review was performed using the PubMed and Scopus databases. Randomized controlled trials, meta-analyses, and guidelines published between 2015 and 2025 were included. Results. PAH diagnosis is based on invasive hemodynamic criteria; however, treatment strategy depends on multidimensional risk stratification. Early combination therapy reduces the risk of clinical worsening by up to ~50%. Prostacyclin pathway agents have the greatest impact on survival, particularly in high-risk patients. Soluble guanylate cyclase stimulators represent an effective alternative in cases of inadequate response. Novel therapies, especially sotatercept, significantly reduce the risk of clinical worsening (up to 84%) and may modify disease progression. Prognosis depends on dynamic treatment response, right ventricular function, and individual patient factors. Conclusions. PAH management is shifting toward an individualized model based on risk assessment and combination therapy. Emerging therapies may significantly improve long-term outcomes; however, further studies are required.
