Abstract [eng] Introduction. Heart failure with preserved left ventricular ejection fraction (HFpEF) is a heterogeneous clinical syndrome that accounts for approximately half of all heart failure cases and is characterized by diagnostic complexity and limited therapeutic options. Aim. To conduct an analysis of the scientific literature and to review the diagnosis and management of HFpEF. Methods. A literature search was performed by analyzing the reference lists of review articles and by using the PubMed database, applying the previously specified keywords. Results. Normal values do not exclude HFpEF because natriuretic peptide levels may remain within the normal range particularly in those with obesity. To improve diagnostic accuracy, additional methods such as cardiac imaging, exercise testing, right heart catheterization, and validated diagnostic scores (H₂FPEF, HFA-PEFF) are used. Effective management of comorbid conditions is a cornerstone of HFpEF treatment. Pharmacological therapy is primarily based on SGLT2 inhibitors, which have been shown to significantly reduce the risk of hospitalization and cardiovascular mortality. Diuretics are prescribed to relieve symptoms and signs of congestion. GLP-1 receptor agonists may be considered in patients with HFpEF and coexisting obesity. Conclusions. The diagnosis of HfpEF is based on typical clinical symptoms, a left ventricular ejection fraction ≥ 50%, objective evidence of diastolic dysfunction or elevated end-diastolic pressure, and increased levels of natriuretic peptides, however, normal values do not exclude HFpEF. Effective management of HFpEF should focus on optimal control of comorbidities. SGLT2 inhibitors represent a key component of contemporary HFpEF treatment, significantly reducing the risk of hospitalization and cardiovascular events.
