Abstract [eng] This dissertation presents a retrospective study, which involved 202 patients who underwent surgical treatment for degenerative mitral regurgitation. Conventional surgical mitral valve repair was performed in 108 patients and 94 patients underwent transapical mitral valve repair. The purpose of this study was to evaluate and compare the efficacy and early and long-term postoperative outcomes of conventional and transapical mitral valve repair. This dissertation also demonstrates the long-term durability of the repair in each of the groups and its association with the initial anatomy of the mitral valve. It also investigates the causes of recurrent mitral regurgitation after transapical and conventional mitral repair. The median follow-up for the patients included in this study was 39 months. It was found that the incidence of adverse events in the early postoperative period was lower after transapical mitral valve repair. However, the long-term durability of conventional mitral repair was statistically significantly better compared to the results of transapical repair and did not relate to the initial anatomy of the mitral valve (long-term durability of conventional mitral repair was 90%, while durability of transapical repair (types A and B) – 77%). The best long-term durability of transapical mitral repair was found to be achieved in patients with an isolated prolapse of the P2 segment.
