There are still more questions than answers in the current perioperative fluid therapy. The individualized goal-directed fluid therapy became the prevailing method a decade ago. However, now it is surrounded by the concerns and questions such as whether it is appropriate to maximize the cardiovascular performance. Also, there is a justified concern that it can be associated with the deterioration of body hydration status. The new method – minimal volume loading test (mVLT) – has been recently introduced for the indirect evaluation of the whole-body tissue hydration. It implies administration of consecutive relatively small crystalloid boluses separated by 5 minute periods without fluid. The plasma dilution is employed as the target parameter. The applicability of the noninvasive measurement of hemoglobin (SpHbTM) is discussed. The results of the initial mVLT method validation studies are shortly described along with the hypotheses for the future research. |
