Abstract [eng] Timeous detection of abdominal aortic aneurysm (AAA) enlargement for patients after endovascular repair (EVAR) and the determinants of the enlargement are important to reduce the number of serious complications. Therefore, this was the purpose of the study. In daily clinical practice measurement of the greatest aneurysm diameter after EVAR is a sufficient and reliable radiologic sign to detect AAA enlargement. An additional measurement of aneurysm volume should be considered in cases of clinically irrelevant diameter increase. This may allow an early detection of clinically significant AAA enlargement. Measurement of the volume of aneurysm thrombus after EVAR is not recommended because of having no impact for further treatment in every day practice. However, analysis of thrombus associated biochemical and histological determinants of AAA growth may be valuable. Significantly elevated levels of white blood cells and C-reactive protein after EVAR are reflected on subsequent aneurysm enlargement. Therefore, immunosuppressive medication for patients developing postimplantation syndrome may reduce the risk of aneurysm progression. The study results show that lower density of aortic aneurysm wall on computed tomography angiography is associated with aneurysm enlargement after EVAR. Revealing the risk factors of lower density of aneurysm wall may be useful in choosing treatment modality for AAA.
