Background / Objective. A single intrathecal morphine injection has a good analgesic effect in various types of surgery. Laparoscopic colon surgery is becoming more common and offers a good alternative to open surgery in selected patients. The purpose of this study was to evaluate the intrathecal morphine analgesic effect after laparoscopic colon surgery. Patients and methods. Fourty patients after laparoscopic colon surgery were included in the study. The patients were randomized to receive intrathecal 0.4 mg morphine (IT gr., n = 20) before anesthesia induction or intravenous 0.02 mg/kg/h morphine dose (gr. IV, n = 20). A standard anesthetic technique, including fentanyl, sevoflurane and air in oxygen, was used. The visual analog scale scores for pain and the amounts of the analgesics were recoded 24 h after surgery. Results. The total dose of morphine and ketolgan in the first 24 hours postoperatively were smaller in the intrathecal group than in group IV (p < 0.05). The quality of analgesia in the first 24 h was better in the intrathecal morphine group (p < 0.05). There were no differences between the two groups as regards the returning of bowel function and the length of hospital stay. Conclusion. Intrathecal morphine before anesthesia induction provided a significantly better postoperative analgesia than the intravenous use of morphine and was effective in reducing analgesics requirement during the first 24 h after laparoscopic colon surgery. |
