Abstract [eng] Chronic kidney disease is associated with significant alterations of the arterial wall structure even in young children that may result in increased arterial stiffness. Adult studies have established a clear link between chronic kidney disease, arterial stiffness and target organ damage but the data in pediatric population was equivocal and relevance of arterial stiffness remained unclear. The aim of this thesis is to evaluate the prevalence and risk factors of arterial stiffness measured by pulse wave velocity and its functional consequences on left ventricular geometry and chronic kidney disease progression in children with pre-dialysis stage 3-5 chronic kidney disease. A total of 667 children from the prospective, multicenter Cardiovascular Comorbidity in Children with Chronic Kidney Disease Study were followed for a median of 1.7 years. Pulse wave velocity was increased in 19.2% of children at baseline and did not change significantly over the follow-up. Higher pulse wave velocity associated with younger age, female sex, lower height, tubulointerstitial kidney diseases, higher blood pressure, lower ferritin levels, higher proteinuria, higher low-density lipoprotein cholesterol and lower vitamin D levels. Higher pulse wave velocity associated with left ventricular concentric remodelling but not chronic kidney disease progression. Collectively, these findings provide strong rationale for routine pulse wave velocity in children with advanced chronic kidney disease.
